Cataract and Smart Lens

Intraocular Lens Replacement: Is It Possible?

Intraocular lens replacement is possible in certain situations. This article explains when the lens from a cataract or smart lens operation can be replaced, why a lens is not changed just because a "new model" appeared, and which solutions come first.

Short answer

Yes, intraocular lenses can be replaced when necessary, but this is not a routine procedure. We usually do it when an unexpectedly high refractive error remains. We also do it when you cannot adapt to night-time light effects with a multifocal lens, even after months. A well-working lens is not replaced just because a "new model" is available. Most complaints can be solved in simpler ways, such as dry eye treatment, YAG laser or a fine-tuning laser.

When is intraocular lens replacement needed?

Intraocular lenses are designed to stay in the eye for life, and they do not wear out. For this reason, I do not recommend replacing the lens in a well-seeing eye because "a newer lens is available". Replacement comes into question in the following situations:

  • Unexpectedly high refractive error: A significant error remains despite the lens power calculation, and no other method can correct it.
  • Light effects you cannot adapt to: With multifocal (trifocal) lenses, halos, glare or loss of contrast around lights at night may not fade even after months. In this case, we usually switch to an EDOF or monofocal lens.
  • Lens displacement: Rarely, the lens can shift off-center, or its supports can weaken.

"Intraocular lenses can be replaced, but these are not operations we enjoy. What matters most is to measure very carefully before surgery and to choose the right lens."

Can an intraocular lens be replaced?

In one minute, Prof. Dr. Fırat Helvacıoğlu explains when intraocular lenses can be replaced.

Solutions tried before intraocular lens replacement

One study examined patients with complaints after a multifocal lens. The most common cause of blurred vision was posterior capsule opacification. Residual refractive error and dry eye followed. In 81% of the eyes, simple treatments relieved the complaints. Only 7% needed a lens replacement.

Cause of the complaintSolution
Dry eyeDrops and eyelid care
Posterior capsule opacification (secondary cataract)YAG laser (postponed if lens replacement is being considered)
Small residual refractive errorSurface fine-tuning laser (PRK) or glasses
Refractive shift caused by gel left inside the lensCleaned surgically if noticed early

We presented our results of PRK fine-tuning after multifocal lenses at ESCRS congresses. We also presented the corneal features that lead to dissatisfaction after trifocal lenses.

If you are unhappy with your lens: a step-by-step approach to intraocular lens replacement

Intraocular lens replacement is the last option. A large share of complaints come from causes that simpler methods can solve, such as dry eye, posterior capsule opacification or a small refractive error.

  1. Intraocular lens exchange – step 1: Finding the cause | Prof. Dr. Fırat HelvacıoğluDry eye, clouding of the posterior capsule, residual prescription and yellow spot problems are investigated one by one. 2. Intraocular lens exchange – step 2: Simple solutions | Prof. Dr. Fırat HelvacıoğluDry eye treatment, YAG laser if needed, or fine-tuning laser on the surface for residual prescription is performed. For a residual error, we use a surface fine-tuning laser.
  3. Intraocular lens exchange – step 3: Lens exchange | Prof. Dr. Fırat HelvacıoğluIf there are intolerable light reflections or a large prescription error, the lens is exchanged while the capsule is preserved. Why is the timing of intraocular lens replacement important?

Replacement is easier and safer in the first months, while the lens has not yet adhered to the capsule in the eye. Over time, the capsule hardens around the lens. Also, once a YAG laser has opened the posterior capsule, removing the lens safely becomes harder. Therefore, if you have complaints after a multifocal lens, we advise against a YAG laser until the replacement decision is final.

In a series of 50 eyes, average patient satisfaction rose from 1.2 to 3.8 out of 5 after lens replacement. So in the right patient, replacement is an effective solution. However, like every intraocular surgery, it has its own risks. It also requires careful surgery that preserves the capsule.

The real solution: correct planning

The way to minimize the need for replacement is to take careful measurements before surgery and to discuss expectations honestly:

  • Evaluation of corneal topography, astigmatism and corneal surface quality
  • Measurement of eye length with optical biometry and lens calculation with current formulas
  • Treatment of dry eye, if present, before surgery
  • Choice of lens type according to your profession and night driving habits

I explained the difference between trifocal and EDOF lenses in the post "Trifocal or EDOF?".

Frequently asked questions

Yes, it can be replaced when necessary. However, the lens does not deteriorate, so this is rarely needed. The most common reasons are an unexpectedly high refractive error and failure to adapt to a multifocal lens.

First, we need to find the cause of your complaint. Causes such as dry eye, posterior capsule opacification or a small refractive error can improve with simple treatments. Also, it takes time for the brain to adapt to the new vision in the first months.

Adapting to multifocal lenses can take a few months. If complaints continue despite this, we plan the replacement while the capsule has not yet hardened, so if possible within the first months.

Yes. Intraocular lenses do not age, and they do not need to be replaced.

We do not recommend replacing the lens in a well-seeing, problem-free eye only for a newer model. The risks of surgery outweigh the benefit you would gain.
